About the Cardiac Nurse Specialist Position
HopeHealth Visiting Nurse is seeking an experienced RN to join and support the clinical team as Cardiac Nurse Specialist.
The Cardiac Nurse Specialist provides support and collaboration to the clinical team for patients identified as high risk, cardiac patients, and functions as a liaison between HHVN and cardiology physician practices to assure that patients receive immediate, appropriate care as needed.
What will you do?
Duties / Responsibilities:
Provides clinical oversight to identified high risk cardiac patients in collaboration with physician practices and HHVN clinical team.
Makes clinical recommendations to physicians for patients requiring revised medication and care delivery regimes to minimize patient Emergency room visits and hospital admissions.
Reviews telehealth alerts as requested by clinical team and elicits telehealth video visits as needed.
Works collaboratively with RN Case Manager and other members of care team to establish and implement patient's plan of care.
Will visit high risk, cardiac patients when needed to provide direct clinical care
Assists with the implementation and education on cardiac disease specific topics for the HHVN clinical staff
Participates in patient case conferences with staff and Clinical Managers.
Recommends changes in utilization of services as well as adjustments to the patient's plan of care to ensure high-quality, cost-effective care with goal on minimizing patient hospitalizations and emergency department use.
Serve as a resource for cardiac disease education for the clinical team
Attends weekly Palliative IDT meetings to discuss complex patients and make recommendations.
What are the qualifications for this job?
Education / Experience:
RN BSN Preferred.
Licenses to practice nursing in the state of Rhode Island and Massachusetts
Minimum of 3 years nursing experience; Minimal of 2 years' experience in a health care setting where job focus was on managing patients with cardiac diagnosis
Nursing Heart Failure certification preferred but not required
Home care experience preferred but not required
Excellent clinical, critical thinking and assessment skills
Strong interpersonal and communication skills
Ability to work independently and be a team player in a collaborative environment.
Physical Efforts and Skills:
Sits up to six (6) to seven (7) hours and stands and walks intermittently throughout the day.
The position requires an ability to occasionally move cartons up to thirty (30) pounds.
Maximum amount would not exceed fifty (50) pounds.
Visual and hearing acuity required.
Ability to drive car.
